The Service Level Requirements Template is used by individuals and entities as a framework to formally define the level of service between parties where one is providing the service and the other is receiving the service. The purpose of this template is to identify the business requirements for system service continuity, availability and performance. The information obtained from this document will be used to produce a Service Level Agreement.
